Requesting a Vehicle De-Insured Certificate is a quick and simple process:

Before you start, you must:

  • Have your Vehicle ID Number (VIN).
  • Have your Zip Code.
  • If this vehicle is registered to an individual, have your Customer/Driver License Number or Customer Name .
  • If this vehicle is registered to a company, have the Company Name and the company's Customer Number.

By submitting this request, you are certifying that:

  • You are the owner/lessee or controlling operator of this vehicle.
  • The vehicle will not be operated on any public roadway in Arizona at any time after submitting this request.
  • The vehicle will not be operated in Arizona until proof of insurance is on file with the Motor Vehicle Division. This may be completed at any MVD or Authorized Third Party provider.
  • You understand that you must still pay any registration fees and taxes required.
